Emergency Help for Wisdom Tooth Pain

Wisdom teeth are the last to come through, and they frequently cause trouble. Pain from an erupting, impacted, or infected wisdom tooth can be severe enough to disrupt your day, and the location at the back of the mouth makes the area prone to infection and swelling.

Common Causes of Wisdom Tooth Pain

Most wisdom tooth pain falls into a few categories. Pericoronitis is the most common: the gum flap covering a partially erupted wisdom tooth traps food and bacteria, causing infection and inflammation. This produces throbbing pain at the back of the mouth, difficulty opening the jaw, swelling, and sometimes a bad taste. Impaction occurs when the wisdom tooth is angled against the tooth in front of it, creating pressure pain and sometimes damaging the neighbouring tooth. Decay can also develop on wisdom teeth because they are difficult to clean properly, and the resulting toothache is often severe because patients do not notice the cavity until it is deep.

Wisdom Tooth Pain and When to Seek Help

Mild discomfort from an erupting wisdom tooth can sometimes be managed at home with thorough cleaning, salt water rinses, and over-the-counter pain relief. But certain signs mean you should call for an assessment: pain that is getting worse rather than better over two to three days, difficulty opening your mouth, swelling that extends beyond the immediate area, pus or a foul taste, fever, or difficulty swallowing. These can indicate pericoronitis or a developing abscess that needs professional treatment. Repeated episodes of pericoronitis are a strong indicator that the wisdom tooth should be removed.

Assessment and Treatment Options

The dentist will examine the area, take an X-ray (often a panoramic X-ray to see all four wisdom teeth and their relationship to the jaw nerve), and determine the best course of action. For acute pericoronitis, treatment involves irrigating beneath the gum flap, prescribing antibiotics if infection has spread, and pain management. Once the acute phase settles, the dentist will discuss whether the wisdom tooth should be removed. Removal of impacted wisdom teeth is a surgical procedure that may be performed by a general dentist or referred to an oral surgeon, depending on the complexity. Most straightforward wisdom tooth extractions can be done under local anaesthetic.
